Preventive Care
Preventive Care at the 23rd Medical Group - Moody Air Force Base consists of immunizations. Our highly-regarded staff and faculty provide clinical excellence and compassionate care to prevent illness, disease, and other health problems for active duty service members and TRICARE beneficiaries.

Immunizations
We are an outpatient clinic offering walk-in immunizations services to active duty service members and TRICARE beneficiaries. A military identification card is required for all services.
- Walk-in hours are Monday–Friday,
7:30 a.m. to noon and 1-4:30 p.m.
- COVID-19 Vaccine: Available to all eligible TRICARE beneficiary categories 12 years old and older.
- Influenza Vaccine: Available to all eligible TRICARE beneficiary categories age 6 months and older.
New patients must drop off immunization records at the immunization clinic. The immunization team has 48 hours to transcribe records into MHS GENESIS.
Patients can walk into the immunizations clinic for well-child visits after same-day PCM appointments. If patients can’t walk into the immunizations clinic for a same-day visit, they must schedule a future appointment.
All appointments can be scheduled by calling the appointment line at 229-257-APPT (2778), selecting Option 1, and selecting your PCM Clinic, or on the MHS GENESIS Patient Portal.
State of Georgia Form 3231
Due to changes in our electronic record system, we are unable to print State of Georgia Form 3231 directly from the site. Patients can request a completed State of Georgia Form 3231 through the MHS GENESIS Patient Portal, or by calling the appointment line at 229-257-APPT (2778), selecting Option 1, and selecting your PCM Clinic.
*Inform the clerk you need to leave a message for the immunizations clinic to get a Georgia Form 3231. *Georgia Form 3231 will be ready to pick up at your PCM Clinic three duty days after request.
Travel Medicine Consults
Any active duty service member or beneficiary traveling abroad for recreational purposes must contact Public Health at least two weeks before departure to have a travel briefing and get scheduled for potential immunizations and medications to assist in a safe trip.
*All Travel Vaccines (Typhoid, JEV, Yellow Fever, etc.) will require a scheduled Immunizations appointment.
